The community is interested in contracting with a provider for the provision of beautician, manicurist, and or barber. This is a 1099 job, you will be required to have a professional liability insurance through an insurance carrier licensed in your application state of no less than 100k per occurrence. Qualifications

• Must have professional liability insurance
• Must have an active provider license in Arizona
• Must show ability to read and write in a legible and understandable manner
• Must speak and understand English
• Positive attitude toward the elderly
